Referrals must be entered via this form PRIOR to joining Physiq Fitness to be eligible for this offer. The promotion period is 1/1/2025 to 2/28/2025 and referrals must be received and join by 02/28/2025. Referred members must make 3 monthly payments before qualifying as a referral.
