Referrals must be entered via this form PRIOR to joining Physiq Fitness to be eligible for this offer. The promotion period is 01/01/2026 to 02/28/2026 and referrals must be received and join by 02/28/2026. Referred members must make 3 monthly payments before qualifying as a referral.
