Referrals must be entered via this form PRIOR to joining Physiq Fitness to be eligible for this offer. The promotion period is 03/01/2026 to 04/30/2026 and referrals must be received and join by 04/30/2026. Referred members must make 3 monthly payments before qualifying as a referral.
