Referrals must be entered via this form PRIOR to joining Physiq Fitness to be eligible for this offer. The promotion period is 5/1/2025 to 6/30/2025 and referrals must be received and join by 06/30/2025. Referred members must make 3 monthly payments before qualifying as a referral.
